Resources

Seniors in Alberta can access a wide range of health and personal care services through the Home and Community Care Program. Services may include nursing services, rehabilitation services, personal care, home support services, and respite care.  The type and level of services provided are based on each client’s unique needs. Seniors can receive these services in the comfort of their homes or other residential settings.

Types of Home and Community Care

Home and community care services are available through three types of providers:

·         Regional Health Authority: Services are provided directly by Alberta Health Services (AHS)

·         Contracted Care: Services are delivered by providers who have agreements with the Regional Health Authority

·         Client Directed Care: Clients choose and hire their own care provider from an approved list.

Seniors can receive services from one or multiple types of home and community care providers simultaneously, depending on their individual needs.

CDHCI program is one of the service models under the Client Directed Care program. It is designed to give seniors greater flexibility and control over their care, allowing them to select and hire their own care provider from an approved list. Impact Alliance Care Services (IACS) is an approved provider, offering personal care services, home support services, and respite care.

To find out if you are eligible for the CDHCI program or to request a home and community care assessment,  please contact Continuing Care Access  or your Alberta Health Services Case Manager.
